Notice Under the Affordable Care Act (ACA), Section 1557

In accordance with the requirements of Patient Protection and Affordable Care Act of 2008 (“ACA”), CODAC Behavioral Healthcare will not discriminate on the basis of race, color, national origin, sex, gender identity, sexual preference, age, or disability.

Please know that in addition to the requirements set forth by the ACA, pursuant to CODAC Behavioral Healthcare policy, CODAC Behavioral Healthcare will provide outpatient, and emergency services without regard to age, race, creed, religion, color, national origin, ethnicity, sexual orientation, pre-disposing genetic characteristics, marital status, gender, disability, familial status, pregnancy, arrest record, conviction record, genetic information, gender identity, care-giving responsibilities, domestic violence victim status, protected veteran status, source of payment, and any and all other protected categories.

Limited English Proficiency: CODAC Behavioral Healthcare will generally, upon request, provide appropriate aids and services so that patients with limited English proficiency can participate equally in CODAC Behavioral Healthcare’s programs, services, and activities. CODAC Behavioral Healthcare will not place a surcharge on an individual to cover the cost of providing any such auxiliary aids or services. Notice Under the Americans with Disabilities Act (ADA)

In accordance with the requirements of title II of the Americans with Disabilities Act of 1990 (“ADA”), CODAC Behavioral Healthcare will not discriminate against qualified individuals with disabilities on the basis of disability in its services, programs, or activities.

Employment: CODAC Behavioral Healthcare does not discriminate on the basis of disability in its hiring or employment practices and complies with all regulations promulgated by the U.S. Equal Employment Opportunity Commission under title I of the ADA.

Effective Communication: CODAC Behavioral Healthcare will generally, upon request, provide appropriate aids and services leading to effective communication for qualified persons with disabilities so they can participate equally in CODAC Behavioral Healthcare’s programs, services, and activities, including qualified sign language interpreters and other ways of making information and communications accessible to people who have speech, hearing, or vision impairments.

Modifications to Policies and Procedures: CODAC Behavioral Healthcare will make all reasonable modifications to policies and procedures to ensure that people with disabilities have an equal opportunity to enjoy all of its programs, services, and activities. For example, individuals with service animals are welcomed in CODAC Behavioral Healthcare offices, even where pets are generally prohibited.

Anyone who requires an auxiliary aid or service for effective communication, or a modification of policies or procedures to participate in a program, service, or activity of CODAC Behavioral Healthcare, should contact the institution’s Civil Rights Coordinator at (401) 275-5037 as soon as possible but no later than 48 hours before the scheduled event. Please note that the ADA does not require the CODAC Behavioral Healthcare to take any action that would fundamentally alter the nature of its programs or services, or impose an undue financial or administrative burden.
